1. A Completed Order Form for our services.
Please ensure that you complete this in full: we use this
form for your contact details, delivery requirements and payment
details.
2. A UK Passport Application Form Completed & Signed by the Applicant.
Forms are available free of charge from any Post Office;
alternatively call us to arrange for some to be dropped off at your
office.
You must use these original forms; photocopied or faxed forms will not be accepted.
Instructions about how to complete the Form are included in the envelope - please follow them very closely.
In particular, please ensure that any signatures are kept
inside the boxes supplied, and if your application requires a
countersignatory (Section 10 of the form) this must be completed, in
full, by the countersignatory only.
3. Two Recent, Colour, Photographs.
We would like to stress the need to
pay
particular attention to the requirements concerning the photographs you
supply - these are enforced strictly, and without
discretion, by the Passport Agency. If your application requires
a countersignatory, the photograph signed as a true likeness of
the applicant by the countersignatory must be done so on
the reverse of the photograph.
3. A Letter of Authorisation to Collect the Applicant's Passport.
This
must be addressed to the UK Passport Agency, Globe House, 89 Eccleston
Square, London SW1V 1PN, and give signed & dated
Authorisation from the Passport
Applicant for "DJB Passports & Visas, Agent 11a" to collect their
Passport on their behalf.
4. For any Applications relating to a Lost or Stolen Passport, A 'LS01' Form, Completed & Signed by the Applicant.
The
'LSO1' form for Replacement of Lost or Stolen Passports needs to be
supplied in addition to the standard Application form where the
application is for the replacement of a Lost or Stolen Passport.
As well as the Post Office and ourselves, this from can be downloaded from the Passport Agency's website
here to be printed and filled in by hand.
